Authorisation for use
of medical information

At Listen Health Pty Ltd (“Listen Health”, “we”, “us”, or “our”), we take your privacy and the protection of your personal health information seriously. This Authorization for Use of Health Information (“Authorization”) explains how and why we may use certain health-related information you provide to us — beyond what is required to deliver our core health services — and asks for your consent to do so.

You are not required to provide this Authorization. If you choose not to, Listen Health will still provide your membership services in the same way.

1. What does “health information” include?
This Authorization applies to your personal health information as defined under the Privacy Act 1988 (Cth). This includes, but is not limited to:
- Your name, date of birth, contact and demographic details
- Your laboratory results, biomarker data, and any other test information collected or received by Listen Health
- Any information relating to your physical or mental health, medical history, or lifestyle factors that may be relevant to your care

2. What is the purpose of this Authorization?
The purpose of this Authorization is to allow Listen Health to use your health information for marketing, educational, and product-related communications that we believe may be useful to you. For example:
- Informing you about new health insights, testing options, or features that relate to your biomarker results
- Providing information about wellness programs, nutrition, supplements, or products that align with your test outcomes
- Sharing relevant news, research, or updates about advances in health and longevity science
Listen Health may also share this information with its related entities and service partners (such as health technology providers, laboratories, or communication partners) for the same limited purposes described above. We will never sell your data.

3. Your rights and important information
- Voluntary Consent: You are not required to sign this Authorization. Choosing not to sign will not affect your ability to use Listen Health’s services.
- Revocation: You may revoke this Authorization at any time by emailing us at privacy@listenhealth.com.au. Your revocation will take effect once received, but any prior use or disclosure made in reliance on this Authorization will remain valid.
- Potential for Redisclosure: Once disclosed, your health information may be subject to further use or disclosure not covered by this Authorization. However, all parties handling your data are bound by Australian privacy laws and Listen Health’s Privacy Policy.
- Access and Copies: You may request a copy of this Authorization or access the health information we hold about you at any time by contacting us in writing.
- Duration: Unless revoked earlier, this Authorization remains valid for five (5) years from the date of your signature below.

4. Acknowledgement and Consent
By ticking this box, you confirm that you:
- Have read and understood this Authorization;
- Authorize Listen Health to use your health information as described above; and
- Understand that you may withdraw your consent at any time without affecting your membership or access to services.
